Exosomes are the newest headline ingredient in aesthetic medicine — and the one most surrounded by hype. They’re genuinely interesting biology, and they’re also the category where the gap between what’s proven and what’s advertised is widest. Here’s an honest primer.
What an exosome is
Cells talk to each other. One of the ways they do it is by packaging molecules into tiny membrane-bound bubbles — extracellular vesicles — and releasing them. Exosomes are the small end of that family, roughly 30–150 nanometers across: far smaller than a cell, far larger than a single molecule.
Their cargo is the point. An exosome can carry proteins, lipids, and RNA — signalling material that influences how recipient cells behave. In regenerative medicine, the interest is straightforward: if you could deliver the messages that healthy, young cells send to each other, you might support tissue repair without transplanting any cells at all.
Why skincare cares
Skin renewal is a coordinated conversation between keratinocytes, fibroblasts and immune cells. Exosome products are used in the hope of supporting that conversation — particularly in the window right after a procedure, when skin is actively rebuilding.
In clinics you’ll typically encounter exosomes:

- Other sources, including cultured cell lines developed by individual manufacturers.
Two questions separate serious products from marketing: what’s the source and purification standard, and how is the material stabilized (exosomes are fragile — handling, cold chain and formulation matter). Polynucleotides are a structural, tissue-conditioning material with a longer track record, used injectably across a course of sessions. FAQ
Are exosome skincare creams the same as clinic exosome treatments?
No. Retail “exosome” creams face the same delivery problem as topical polynucleotides — intact skin is a very good barrier. Clinical use pairs exosomes with a procedure that temporarily opens that barrier, which is a different proposition entirely.
Do exosomes contain stem cells?
No — and that’s the appeal. Exosomes are vesicles released by cells; they carry signalling cargo but are not living cells and don’t replicate. Products derived from stem-cell cultures contain the vesicles, not the cells.
Are exosome treatments approved?
It depends on the country, the source and the intended use, and the regulatory landscape is actively changing. Ask your provider what specific product they use and what its status is where you’re being treated.
Should I choose exosomes or polynucleotides?
That’s a consultation question, not a shopping question. They address different aspects of skin renewal, and experienced practitioners often combine them within one treatment plan.
